I certify that all information provided by me in the membership application is true and correct. I further certify that I have read and understood the Means of Conduct and the document containing qualifications to apply for the membership of the institution and that I agree to abide by same. I further state that I am truly aware that if the institute becomes aware that the information provided by me in the membership application is false and if by any action of mine any objectives / policies of the institution have been violated, my membership stands terminated and that the authority lies with the institute and its Board of Directors to terminate my membership.
Further, I am aware that accepting or rejecting the application that I submit to obtain membership lies with the Institution. I being properly informed and having understood the above placed my signature hereunder.
